Information we collect

Application information: name, contact details, school, personal statement, transcript, and recommendation letter
Compliance documentation submitted to the secure portal: training certificates, waivers, immunization records
Mentor and host practice contact and affiliation details
Donation records processed by our payment provider; we do not store card numbers
Basic website analytics such as page views and referring source

Use & storage

How information is used

Information is used to evaluate applications, coordinate placements, meet compliance obligations of host practices, track program outcomes, and communicate with scholars, mentors, and supporters. We do not sell personal information, and we do not share it with third parties except service providers acting on our behalf or where required by law.

Compliance documents are stored in the authentication gated document center with role restricted access. Outcomes and survey data used in public reporting is aggregated and deidentified. Application materials for scholars who are not admitted are deleted after one cycle; scholar records are retained for seven years for outcomes tracking.
